All on-site personnel are included in the corporate medical monitoring program and <br /> receive physical examination on an annual basis. <br /> Additional medical monitoring may be required if exposure or potential exposures are <br /> identified by the FSS for which such monitoring is deemed necessary. The content <br /> of this monitoring will be determined by the FSS and consulting physician. <br /> The petrochemicals typical of these facilities can affect specific organ systems <br /> producing characteristic health effects. The medical evaluation will, therefore, focus <br /> on the liver, kidney, nervous system, blood systems, skin and lung functions. <br /> Laboratory testing will include complete blood count and applicable kidney and liver- <br /> function tests. Other tests include skin examination. <br /> 15.2 Training Requipements <br /> All personnel (including subcontractors) whose work requires entry into exclusion <br /> zones, decontamination areas or otherwise present potential exposure to health or <br /> safety hazards associated with this project, shall meet the training requirements set <br /> forth in 29 CFR 1910.120, the OSHA Hazardous Waste Operations and Emergency <br /> Response Standard. <br /> Personnel shall not be permitted to participate in or supervise activities in the <br /> exclusion or decontamination zones until they have completed the specified training. <br /> Written documentation of the completion of training (e.g., copies of course certificate) <br /> must be submitted to the FSS prior to entry into work areas. <br /> Specifically, such personnel must complete: <br /> • 40 hours of off-site instruction on the health and safety topics prescribed in 20 <br /> CFR 1910.120. <br /> • 8 hours of refresher training within the past 12 months <br /> • Site specific training <br /> • Prior to the initiation of sampling activities, all personnel shall attend a tail- <br /> gate safety meeting conducted by the FSS. The training shall address the <br /> following: <br /> 1. The content of this HASP, with emphasis on the hazards specific to <br /> this site, work rules, standard operating procedures, PPE requirements <br /> and decontamination. <br /> 2.
