3.0 FIELD ACTIVITIES <br /> and 10 feet below ground surface (bgs) and exhibited strong petroleum hydrocarbon odor. <br /> Both samples were labeled and placed in a cooler with ice. All samples collected during the <br /> project were delivered via courier to APPL Laboratories, Inc.in Fresno,CA.The disposal <br /> pre-acceptance sample results were submitted to Forward Landfill and are discussed in <br /> Section 3.7.1, and analytical results are presented in Table 1. <br /> 3.3 Pre-construction Monitoring Well and Piezometer <br /> Abandonment <br /> Monitoring well MW-2 (located on the northeast end of the excavation) was abandoned on <br /> October 11,2004 by over-drilling the entire well to 22 feet bgs and then tremie grouting with <br /> neat cement to the surface. Piezometers PZ-2 and PZ-3 were also abandoned on October 11, <br /> 2004 by pressure grouting the polyvinyl chloride(PVC)casing in place and cutting it flush <br /> with the ground surface.The piezometer structures were subsequently removed during <br /> excavation activities. On November 2,2004 piezometer PZ-4 was removed by first grouting <br /> the casing and then removing it with a backhoe. The monitoring well and piezometers were <br /> abandoned under permit and oversight of a representative from San Joaquin Environmental <br /> Health Department. A copy of the approved well destruction permits and well closure <br /> records are included in Appendix C. <br /> 3.4 Site Preparation/Demolition <br /> Site preparation activities included mobilizing construction equipment,setting up a soil <br /> staging area, and installing temporary security fencing. <br /> The excavation area was properly marked,and Underground Service Alert was notified a <br /> minimum of 48 hours prior to initiating the fieldwork. In coordination with PG&E,a private <br /> underground utility locating service was retained to survey and mark underground utilities <br /> within the construction area prior to commencing work. <br /> Arrangements were made with UPRR for crossing over the railroad tracks as the <br /> undercrossing cannot accommodate large trucks. A grading permit was obtained from the <br /> San Joaquin County Community Development Department(Appendix B). <br /> A decontamination zone and a designated staging area were established within the site <br /> construction boundaries. Facilities included a wash area for personnel decontamination <br /> (boot wash and hand-wash basin), a pressure-washing area for equipment leaving the site, <br /> and an area for personnel to don/remove personal protection equipment. An emergency <br /> eyewash station was also located within the decontamination zone. Exclusion zones (soil <br /> handling and dewatering) and support zones (vehicular parking and equipment staging) <br /> were clearly identified and delineated. Site security was provided by a temporary perimeter <br /> fence and locked-gate access on the south side of the site,replacing permanent fencing that <br /> was removed. <br /> EM Assist, Inc. removed remaining inactive underground utilities and piping associated <br /> with former process equipment previously removed by PG&E. Subsurface gas pipelines <br /> were removed and disposed of at an approved non-hazardous waste landfill. Concrete slabs <br /> were broken up and disposed of at a concrete recycler.
