HSC 119315-Invoices and log kept for disposable, pre-sterilized equipment <br /> OBSERVATION: Sterilization certificates were not provided for the single use Emalla piercing needles. <br /> CORRECTIVE ACTION: Records of purchase, a log of all procedures (including names of practitioner and client and the date of <br /> procedure), and sterilization certificates must be maintained for 90 days following the use of the instruments. Submit sterilization <br /> certificates for the Emalla piercing needle. <br /> #9. HSC 119309- No eating,drinking or smoking -clean clothes <br /> OBSERVATION:A microwave and coffee maker were observed near the procedure area sink. <br /> CORRECTIVE ACTION: No food or drinks are permitted in the procedure area. Remove the microwave and coffee maker from <br /> the procedure area. <br /> #18. HSC 119303 -Client records approved and available -Consent form and questionnaire <br /> OBSERVATION: Client records do not document the lot numbers of the disposable instruments used in the procedure. <br /> CORRECTIVE ACTION: Moving forward, the facility shall document all lot numbers for all disposable instruments/needles used <br /> for each procedure/client. <br /> #19. HSC 119303 -Appropriate aftercare instructions given to client <br /> OBSERVATION: The facility does not have after-care instructions for piercings. <br /> CORRECTIVE ACTION: Submit a copy of the after-care instructions for piercings. The instructions should include: care of the <br /> procedure site, restrictions on physical activities, signs &symptoms of infection, and when to seek medical care. <br /> #32. HSC 119314-Floors and walls clean and in good repair, adequate light <br /> OBSERVATION: <br /> 1. Small holes in the wall in Jesus' and vacant procedure area. <br /> 2. Baseboards are not properly installed. Baseboards are coming off, creating gaps between the wall and the baseboard <br /> throughout the procedure area. <br /> CORRECTIVE ACTION: <br /> 1. Seal/repair holes and paint over it with gloss finish paint. <br /> 2. Properly install baseboards to ensure walls are smooth, nonabsorbent, free of open holes and washable. <br /> #33. HSC 119314-Workstation,surfaces, including chairs,armrests, etc. in good repair <br /> OBSERVATION: Absorbent materials were observed in the procedure area. <br /> 1. A cap and hat in Jose's procedure area. <br /> 2. A yellow towel hung on the wall in Victor's procedure area. <br /> 3. Cloth pillow in Jesus' procedure area. <br /> 4.
