Set-up and tear down information was not provided for <br /> piercings and the plan failed to address service animals. <br /> 2. The facility does not have Annual IPCP training records. Per Jesus, he provides training but has does not document <br /> the trainings. <br /> CORRECTIVE ACTION <br /> 1. Submit a revised IPCP to the Environmental Health Department(EHD). <br /> 2. Provide training to facility employees and submit training records to the EHD. <br /> Submit evidence of corrections to the EHD by <br /> 06/08/2025. <br /> NOTES: <br /> 1. Inspection was conducted with REHS Sandip Singh. <br /> 2. Emailed the inspection report to the facility owner Jesus Garibay at onetimetattoostudio(cDgmail.com <br /> 3. Reviewed consent forms, medical questionnaire, and after-care instructions. <br /> 4. Reviewed IPCP.Training is required initially,when there's a change, or at least annually. Keep records for 3 years. <br /> 5. Per the IPCP,the facility will use Repbulic for sharps disposal. Per Jesus, he did not have any disposal records because <br /> he has not had any waste picked-up since the last inspection.We informed Jesus he can only use the mail back system <br /> or have his waste picked up by a certified hauler. <br /> 6. Facility's front sign advertises tattoos and piercings. Per Jesus he provides piercings on occasion, including tongue <br /> piercings. Jesus states he uses single-use presterilized jewelry, needles, and instruments only. He did not have any <br /> available during the inspection because he stated he has not restocked his inventory. Submit photos of the presterilized, <br /> single-use jewelry, needles, and instruments once back in stock. <br /> Reinspeetion ontabout: A reinspection fee of$172 per hour may be charged.
