M) Work tables/trays and chairs or benches shall be decontaminated with a <br /> germicidal solution after each tattoo application. Hard surface disinfection will <br /> consist of PDI wipes. All hard surfaces in the procedure area will be disinfected <br /> after each client. The chairs, counters, etc. will be sprayed with PDI wipes. The <br /> disinfectant will be allowed a contact time of 3 minutes (according to <br /> manufacturer's directions for dilution and contact line), then wiped down with a <br /> paper towel which is disposed of to the trash. <br /> N) All materials applied to the human skin shall be from single use dispensers <br /> transferred from bulk containers to single use containers and shall be disposed of <br /> after each use. <br /> 0) No animals shall be permitted in the procedure area. <br /> 2. Records <br /> A. Records, including a complete informed consent form and post procedure <br /> instructions for all tattoo, brand and piercing administered, shall be maintained by <br /> the holder of the permit. All records will be maintained onsite and available for <br /> review for a period of two (2)years, after which the records shall be shredded to <br /> destroy confidential medical information. <br /> B. A copy of all registrations licenses, and Exposure Control Training for all body <br /> artists employed or contracted to the facility will be maintained on-site, available <br /> for review. <br /> C. Records and procedure logs of all purchased pre-sterilized instruments/parts will <br /> be maintained on-site, available review for a period of 90 days after the <br /> instrument is used. <br /> 3. Informed Consent Form <br /> A. Informed consent form for each client shall be read, completed, and signed prior <br /> to any procedure being performed and shall include all elements of section <br /> 119303 of the Safe Body Art. <br /> 4. Post Procedure Instructions <br /> A. Printed procedure specific instructions on the care of procedure site shall be <br /> given to each patron or customer after tattooing, branding, or piercing as a <br /> precaution to prevent infection. Instructions shall include restrictions on physical <br /> activities such as bathing, recreational water activities, gardening or contact with <br /> animals and the duration of the restrictions, signs, and symptoms of infection and <br /> indicate the need to seek medical care. <br /> 5. Dyes or Pigments <br /> A. In preparing inks, dyes, or pigments to be used by a tattoo artist, only <br /> commercially available single-use or individual portions of dyes or pigments in <br /> clean, single-use containers shall be used for each patron. <br /> B.After tattooing, the remaining unused dye or pigment in the single-use <br /> individual containers shall be discarded along with the containers. <br />
