Each sterilization load shall be monitored with mechanical indicators for time, temperature and pressure, Sterilization shall be <br />accomplished by steam autoclave, manufactured for the sterilization of medical instruments, and pass a Class V integrator test. <br />HSC 119315(b) The facility is using Class IV integrators instead of the required Class V integrators. Only Class V <br />integrators are approved for use. Provide Class V integrators for each cycle/load. <br />4. Sterilizer shall be spore -tested after initial installation, after major repair, and at least once per month. A written sterilization <br />shall be maintained for 3 years. The log shall include spore -test results each sterilization cycle, date, contents, exposure time and <br />temperature, results of the Class V integrator for every cycle/load, and evidence of an acceptable spore test before reuse of the <br />sterilizer after a failed spore -test. HSC 119315(b) 1. No evidence of monthly spore testing available at the time of the <br />inspection. DO NOT USE THE AUTOCLAVE UNTIL EVIDENCE OF A RECENT SPORE TEST HAS BEEN <br />PROVIDED TO THE EHD. 2. The sterilization log was only available for seven months. The facility is required to <br />maintain the last three years of records. 3. The entries in the sterilization log do not list the time and temperature for <br />every cycle/load. Record the time, temperature, and the results of the Class V integrator with each cycle/load. Staple <br />the integrator to the sterilization log on side of the cycle entries. 4. No evidence of sterilization of jewelry in the <br />sterilization log. All contents are required to be logged at the time of sterilization. <br />6. A facility that uses single use instruments shall maintain record of purchase, log of procedures, names of practitioner and <br />client, date of procedure, instruments used, and written evidence from the manufacture that the instruments have undergone <br />sterilization. The record of purchase, evidence of sterilization, type and number of instruments used shall be maintained for a <br />minimum of 90 days after use. HSC 119315(f) Sterilization records were not available for the pre -sterilized lots in use in <br />the facility. Provide sterilization certificates that match lots in use in the facility. <br />7. The sharps waste container shall be within arm's reach and labeled with the word "sharps waste" or with the biohazard symbol <br />and the word "Biohazard". Sharp waste containers shall be disposed by a licensed waste hauler or approved mail back system. <br />Documentation of proper disposal shall be maintained for 3 years. HSC 119314(e) No records available for sharps disposal <br />at the time of the inspection. Full sharps containers were observed during the inspection. Sharps must be removed a <br />minimum of once per year, and a copy of the records must be maintained onsite for review. Provide evidence of a <br />recent sharps disposal. <br />8. Jewelry placed in newly pierced skin shall be sterilized prior to piercing or purchased pre -sterilized. If purchased pre -sterilized, <br />sterile packs shall be discarded if integrity is compromised. Clean calipers shall be used and the skin marked using clean <br />toothpicks and ink or a single -use marking pen. Sterilized instruments shall be stored in sterilization packaging or in sterilization <br />equipment cartridge until time of use. Sterile instrument packs shall be evaluated before use and shall be discarded if integrity is <br />compromised. HSC 119310 No evidence of purchase of pre -sterilized jewelry or of sterilization of jewelry in the <br />sterilization log. Sterilized jewelry must be logged in the sterilization log at the time of sterilization. <br />9. No food, drink, tobacco product, or personal effects are permitted in the procedure area. HSC 119309 Packaging from food <br />and beverages and dog treats were in the procedure area and in trashcans in the procedure area. <br />Reinspecion on/about: A reinspection fee of $152 per hour may be charged.
