<br />18. The facilities client consent form and medical questionnaire must include all of the information listed in Form B, and are
<br />available for inspection. HSC 119303 1. Only one month of records for piercing were available at the time of the
<br />inspection. 2. No records for tattooing were available for review at the time of the inspection. 3. The medical
<br />questionnaire is barely legible and requires updating to ask about cardiac valve disease instead of heart condition,
<br />herpes at the procedure site, allergy to latex, current medications, and other risk factors for bloodborne pathogens.
<br />Provide an updated copy of the medical questionnaire to the EHD.
<br />19. At the completion of the procedure, the practitioner shall provide post-procedure instructions that include all elements listed in
<br />Form B. HSC 119308(b) No post-procedure instructions were available at the time of the inspection. Provide post-
<br />procedure care instructions with all of the elements listed in Form B (see attached). Provide post-procedure
<br />instructions to the EHD.
<br />31. A body art facility shall be separated from any residential areas used for sleeping, bathing, or meal preparation and shall not
<br />share a common entrance or toilet facility with residence. The toilet facility must have a sink with hot and cold water,
<br />containerized liquid soap, and single -use paper towels that are dispensed from a wall -mounted, touchless dispenser. HSC
<br />119314 A blanket for a dog and dog treats were in the procedure area. Dogs are not allowed in the procedure area.
<br />32. A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. The
<br />body art facility shall be free of insect and rodent infestation. The procedure area must be separated, by a wall, ceiling to floor,
<br />from nail and hair activities and be separated from all business not related to body art. Approval of co -located body art facilities
<br />are at the discretion of the LEA. A procedure area must be equipped with adequate light to perform a procedure. Each procedure
<br />area shall have lined waste containers. HSC 119314 & HSC 119317 1. The floor in the procedure area is absorbent. Repair
<br />the absorbent area. 2. The wall in the unauthorized addition is absorbent. Repair the absorbent area. 3. There is an
<br />unlined trashcan in procedure area. Provide a liner for the trashcan. 4. There is a picture in the procedure area with a
<br />hole in it that has to be repaired or removed.
<br />34. Certificate of registration and health permit shall be posted in a conspicuous place. HSC 119306 and HSC 119312 Post the
<br />facility permit, artist permits, and bloodborne pathogen certificates in the facility.
<br />33. All surfaces and objects in the procedure area, including chairs, armrests, tables, countertops, and trays, shall be in good
<br />repair and capable of being disinfected. All counter surfaces and service trays in the procedure area shall have a smooth,
<br />durable, and nonabsorbent finish. HSC 119314 Two stools in the procedure area have absorbent areas. Repair the
<br />upholstery.
<br />35. Operation and employee training records shall be maintained for 3 years and made available for inspection. A written
<br />Infection Prevention and Control Plan (IPCP) shall be maintained, followed and updated. HSC 119313 The IPCP was not
<br />available for review onsite. Provide an updated copy of the IPCP to the EHD.
<br />37. A body art facility shall not conduct business without a valid health permit. A person shall not perform body art without a valid
<br />registration and a permitted facility. A body art facility shall notify the LEA in writing within 30 days of resignation, termination of a
<br />practitioner. A new hire or apprentice must be registered the EHD prior to performing body art at the facility. HSC 119312 & HSC
