Each sterilization load shall be monitored with mechanical indicators for time, temperature and pressure, Sterilization <br />shall be accomplished by steam autoclave, manufactured for the sterilization of medical instruments, and pass a Class V <br />integrator test. HSC 119315(b) <br />The autoclave in use in the facility does not monitor pressure. If the autoclave is in need of repair or replacing, it <br />must be replaced with an autoclave that measures time, temperature, and pressure. <br />4. Sterilizer shall be spore -tested after initial installation, after major repair, and at least once per month. A written sterilization <br />shall be maintained for 3 years. The log shall include spore -test results each sterilization cycle, date, contents, exposure time and <br />temperature, results of the Class V integrator for every cycle/load, and evidence of an acceptable spore test before reuse of the <br />sterilizer after a failed spore -test. HSC 119315(b) <br />-No evidence of spore testing was available onsite. DO NOT USE THE AUTOCLAVE UNTIL EVIDENCE OF A <br />RECENT SPORE TEST IS PROVIDED TO THE EHD. Submit evidence of a spore test to the EHD by 12/11/2019. <br />-No written sterilization log was available at the time of the inspection. Once permission has been obtained from <br />the EHD to use the autoclave, a written sterilization log must be maintained as described above. <br />7. The sharps waste container shall be within arm's reach and labeled with the word "sharps waste" or with the biohazard <br />symbol and the word "Biohazard". Sharp waste containers shall be disposed by a licensed waste hauler or approved mail <br />back system. Documentation of proper disposal shall be maintained for 3 years. HSC 119314(e) <br />No evidence of a sharps disposal since 5/18/17 (Sharps Solutions). Sharps must be disposed of at least once per <br />year. Submit evidence of a recent sharps disposal to the EHD by 12/11/2019. <br />34. Certificate of registration and health permit shall be posted in a conspicuous place. HSC 119306 and 119312 <br />Post Michael Carter's bloodborne pathogens and permit when obtained. <br />37. A body art facility shall not conduct business without a valid health permit. A person shall not perform body art without <br />a valid registration and a permitted facility. A body art facility shall notify the LEA in writing within 30 days of resignation, <br />termination of a practitioner. A new hire or apprentice must be registered the EHD prior to performing body art at the <br />facility. HSC 119312 & 119306 <br />Artist Michael Carter has been practicing in the facility without a permit since September. Mr. Carter must submit <br />evidence of current bloodborne pathogens training and obtain a permit from the EHD by 12/6/2019 or Mr. Carter <br />must cease to practice at the facility and the facility will be charged $152. <br />Reinspecion on/about: A reinspection fee of $152 per hour may be charged.
