Tattoo machine and clip cords: All tattoo machines and clip cords are sprayed down with a cide <br /> and allowed to soak for 3-5 minutes before being wiped down, before and after all tattoos. <br /> 9. Reusable instruments, calipers, needle tubes, etc.,portable light fixtures or other: All reusable <br /> instruments, calipers, and needle tubes are immersed in a cide and allowed to soak for 3-5 minutes after <br /> each use or if their packaging/seal is ripped or opened and then sterilized again. Portable light fixtures <br /> are sprayed with a cide and allowed to soak for three to five minutes before being wiped down at least <br /> once daily <br /> 10. Permanent cosmetic machine: we do not perform any cosmetic tattooing therefore we do not own <br /> any cosmetic tattoo equipment. <br /> B. Reusable Instruments or Disposable: Describe the procedures used for decontaminating, sterilizing, <br /> packaging and storing of reusable instruments. Include the procedures for labeling of sterilized peel- <br /> packs. Indicate whether the body art facility uses all pre-sterilized, single use and disposable <br /> instruments. Describe the record keeping logs and procedure logs maintained on-site when using 100% <br /> pre sterilized, single use and disposable instruments. <br /> 1. Needle tubes: Most needle tubes are single use and pre-sterilized. Once a pre-sterilized needle tube <br /> has been used it is thrown away. All invoices from ordering pre-sterilized needle tubes and needles are <br /> kept on site. The backs of all of the peel packs for pre-sterilized needle tubes and tattoo needles are <br /> attached to the back of the release form after it has been filled out. Every release form is held on site for <br /> at least ninety days and then taken to an off site storage location. Needle tubes that are reusable are <br /> submerged in a cide and allowed to soak for 3-5 minutes before being placed in an ultrasonic machine <br /> after each tattoo. The tubes will run a cycle for a minimum of 60 minutes in the ultra sonic machine in a <br /> solution such as cross-zyme. After the cycle in the ultra sonic, the tubes are manually washed with a <br /> mixture of soap and water and then allowed to air dry. After the tubes are completely dried,they are <br /> individually packaged and labeled with the date, persons initials that prepared them,the load number <br /> and then put into the autoclave for sterilization. Reusable sterilized tubes are stored in sealable/closing <br /> plastic containers in or near the workstation but kept away from the cleaning room. <br /> 2. Calipers: The calipers used at this facility are reusable. After each use, all calipers are soaked in a <br /> cide for 3-5 minutes. After the soaking is complete,the calipers are manually washed with a mixture of <br /> soap and water. After the washing is complete the calipers are rinsed thoroughly and then allowed to air <br /> dry. Once the calipers are completely dried,they are individually packaged and labeled with the date, <br /> persons initials that prepared them,the load number and then put into the autoclave for sterilization. <br /> Reusable sterilized calipers are stored in sealable/closing plastic containers in or near the workstation <br /> but kept away from the cleaning room. <br /> 3. Other instruments: Other instruments include forceps, hemostats, ring opening pliers, ring closing <br /> pliers, and micro dermal forceps. . After each use, all of these instruments are soaked in a cide for 3-5 <br /> minutes. After the soaking is complete, the instruments are manually washed with a mixture of soap and <br /> water.
