0 <br />St. Joseph's Medical Center <br />A member of CHW <br />See document #1 from Healthcare Clinical Lab which shows a positive reading on July 1St, 2011 and July <br />50', 2011. The failure on July 5th° 2011 was an expected failure. <br />See document #2 is an invoice from Regulated Medical Waste for repairs performed on our steam sterilizer. <br />See document #3 which is our certification of calibration from Regulated Medical Waste Industries which <br />was issued April 7th, 2011. <br />See document #4 which is an email from Republic Services (Allied Waste) stating that Allied Waste had <br />been contacted regarding the issue with the sterilizer.
