0 0 <br />St. Joseph's Medical Center <br />A member of CHW <br />The placement of a rigid container in a patient room would not impact this process, as the <br />housekeeping/nursing staff would still remove the bag and take it to the soiled utility room to <br />store/transport. <br />It is our hope that the county health department would excuse this violation based upon the aforementioned <br />clarification of our process. The medical center currently has not relocated the containers, as we would <br />prefer to discuss in greater detail. <br /># 12 — A treatment record is missing for the week of March 2 through the 91h for 2011 <br />Treatment records must be kept onsite and available for inspection for a minimum of 3 years. <br />• Submit to the EA the missing treatment record within 30 days. <br />We are unable to locate/produce the treatment record for the week of March 2nd — March 9", 2011. <br />#16 — Spore testing performed in July 2011 came back positive indicating inadequate sterilization. <br />A spore test performed on July 1St came positive for the bacteria on the 2nd of July. A second spore <br />test performed on July 7 came back positive for the bacteria on the 11th of July. Spore tests <br />performed on July 5th and the 13th came back negative for the bacteria on the 12th and the 201h of <br />July. An invoice given to me by this facility shows service was performed on the autoclave on the <br />18t of July. This facility could not ensure that medical waste treated onsite was adequately <br />sterilized until the 20th or at the every least the 12th of July. All medical waste generated and <br />required to be disposed between the dates of July 2"d and the 1 lth should have been hauled offsite by <br />a medical waste hauler to a permitted medical waste treatment facility. Documents obtained from <br />forward landfill show this facility had medical waste hauled offsite by a solid waste hauler to a solid <br />waste disposal facility. The dates the medical waste was hauled off are on July 4th, 13th, and the <br />18th. <br />Prior to disposal, this Large Quantity Generator must ensure medical waste is treated in accordance <br />with Chapter 8 Section 118215 (a) (2) <br />• Provide to this Enforcement Agency a corrective action detailing the measures that will be <br />put in place to show this facility's response to inadequate sterilization. Provide the <br />corrective action letter for review within 30 days. <br />While attempting to explain/walk through this issue with the inspector, it became apparent that our <br />explanation was disregarded, thus leading to frustration from the inspector as well as the medical center. <br />We then asked that the conversation/inspection end and that the inspector document his <br />finding/concerns/perceptions and forward to us for an official response. <br />
