This registered large medical waste generator does not maintain individual treatment records and tracking <br /> documents for 2 years. <br /> -A large medical waste generator required to register with the EHD shall maintain individual treatment operation <br /> records and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br /> HSC117975(a)— Several treatment records from Daniels Sharpsmart Inc. do not have a signature under the <br /> "certificate of destruction section." View treatment records prior to printing to ensure treatment of waste has <br /> been verified. Speak with hauler regarding treatment records that do not have a verification of treatment. <br /> Submit evidence of compliance by 1/9/2016. <br /> 24. Pharmaceutical waste is not segregated for storage. <br /> - Pharmaceutical waste shall be segregated for storage in accordance with the facility's medical waste management <br /> plan. Shipping container shall be in compliance with DOT and DEA. HSC 118275(a)(6) <br /> -Nonradioactive and non-RCRA pharmaceutical wastes,that are regulated as medical waste, shall be placed in a <br /> container or secondary container labeled with the words "HIGH HEAT" OR"INCINERATION ONLY"on the lid <br /> and sides, so as to be visible from any lateral direction. HSC 118275(a)(6)(B)—Pharmaceutical waste container <br /> located in the soiled utility room (2°d floor north) did not have a label on top. Label container and submit <br /> evidence of compliance by 1/9/2016 <br /> 25. Biohazardous waste is stored above 32 F onsite for up to 30 days before removal. <br /> -A person who generates 20 or more pounds of biohazardous waste per month shall not contain or store that waste <br /> above 32 F onsite for more than 7 days. A person who generates less than 20 pounds of biohazardous waste per month <br /> shall not contain or store that waste above 32 F onsite for more than 30 days. HSC 118280(e)(1)—Either have <br /> hauler remove waste once every 7 days or submit evidence to show that less than 20 pounds of biohazardous <br /> waste is generated to be treated offsite. Sumbit evidence of compliance by 1/9/2016. <br /> 26. Sharps waste is not placed in sharp container. Sharps container is not handled and/or labeled appropriately. <br /> - Sharps waste shall be placed in a sharps container. Full sharps container ready for disposal shall be taped closed or <br /> tightly lid and stored onsite not more than 30 days. Sharps container shall be labeled with the words"Sharps Waste" <br /> or BIOHAZARD". HSC 118285—Biohazardous waste container placed in soiled utility room in the ICU had a <br /> glass bottle inside. Glass bottles should be placed in a sharps container. Train staff and submit evidence of <br /> compliance by 1/9/2016. <br /> Reinspecion on/about: A reinspection fee of$130 per hour may be charged.
