HSC 118307–The janitor's closet and soiled utility room on the <br /> 1St floor(same day surgery) did not have a biohazard label on the door. Add label and submit evidence of <br /> compliance by 1/9/2016. <br /> Notes: <br /> 1.) Facility currently contracts with Stericycle for the removal of Pathology and Chemotherapy waste. <br /> Facility contracts with Daniels Sharpsmart Inc for the removal of biohazardous and pharmaceutical <br /> sharps waste. Note that there are a few small sharps containers onsite that do not belong to Daniels. <br /> These are treated in the sterilizer onsite. Ensure sharps containers are placed so that labels are visible. <br /> 2.) Facility has a Sani-Pak sterilizer onsite. Records of sterilization were available for review. Records of <br /> spore testing were available for review. Records of calibration were available for review <br /> 3.) Biohazardous waste bags used to collect waste that will be treated onsite must meet ASTM D1709. <br /> Biohazardous waste bags used to collect waste that will be sent offsite for treatment must meet ASTM <br /> D1709 and D1922. <br />
