SCOPE <br />This policy applies to the Sutter Valley Area (SVA) composed of: <br />• Memorial Hospital Los Banos <br />• Memorial Medical Center <br />• Sutter Amador Hospital <br />• Sutter Auburn Faith Hospital <br />• Sutter Center for Psychiatry <br />• Sutter Davis Hospital <br />• Sutter Medical Center Sacramento <br />• Sutter Roseville Medical Center <br />• Sutter Solano Medical Center <br />• Sutter Tracy Community Hospital <br />DEFINITIONS <br />None. <br />RESPONSIBILITY AND AUTHORITY <br />A. The Governing Board: Commitment to and financial support for provision of a safe environment of care <br />consistent with the mission, services, laws and regulations. Delegates program management <br />responsibilities to hospital administration. <br />B. Administration: Commitment to Hazardous Materials and Waste Management Program is demonstrated <br />through policy statement, delegation of program management to the Safety Coordinator and/or designee <br />to ensure program effectiveness. <br />C. The Hazardous Waste Coordinator is appointed to assist with the development and implementation of the <br />affiliate program. Duties will include: <br />1. Being a standing member of the affiliate Environment of Care/Safety Committ <br />ee, <br />2. Functioning as the liaison between waste concerns and the Environment of Care/Safety Committee, <br />3. Providing regular reports to the Environment of Care/Safety Committee, <br />4. Providing a written quarterly Environment of Care Report to the Environment of Care/Safety <br />Committee, <br />5. Providing an annual Environment of Care Report of Hazardous Materials and Waste Management <br />Program to the Environment of Care/Safety Committee, <br />6. Serving as a resource to staff. <br />7. Managing the 3E database. <br />D. The Radiation Safety Officer has primary responsibility for the radioactive waste program. <br />E. The Environmental Services Manager or designee has primary responsibility for medical/infectious <br />wastes, including sharps and has primary responsibility for non-RCRA (Resource Conserv <br />ation and <br />Recovery Act) pharmaceutical and trace chemotherapy waste. <br />F. The Hazardous Waste Coordinator or designee has primary responsibility for bulk RCRA <br />chemotherapeutic and hazardous chemicals and wastes. <br />Hazardous Materials and Waste Management Plan.
