For <br /> example violation number 2 was for having biohazard labels on the normal trash containers throughout the <br /> clinic. What I would need is photographs of the containers in the dental clinic showing that the labels have been <br /> removed. As for violation 9 we would need copies of the sterlization records. Gina stated that they were working <br /> on transferring the records from the autoclave onto a computer for easier review. Please email or fax the records <br /> to me. For the spore tests we would want to see test results showing adequate sterlization. You can take a <br /> photograph of the results or if there is a way to print results from the machine you can do that and fax or email <br /> them over to me. For the issue regarding the segregation of pharmaceutical waste, photographs as well as a <br /> corrective action plan, or any kind of training documents given to staff will suffice.
