Certainly any documentation we have to substantiate our actions will be sent to
you. My main concern was for proof of labeling, since we are not permitted to carry cameras. I will see what we
can do to arrange for that.
Thank you,
Stephanie Rising
Standards Compliance Coordinator
California Health Care Facility-Stockton
209-467-4657
stephanie.risinc(a cdcr.ca.gov
SERVICESHEALTH CARE
From: Harprit S. For
example violation number 2 was for having biohazard labels on the normal trash containers throughout the
clinic. What I would need is photographs of the containers in the dental clinic showing that the labels have been
removed. As for violation 9 we would need copies of the sterlization records. Gina stated that they were working
on transferring the records from the autoclave onto a computer for easier review. Please email or fax the records
to me. For the spore tests we would want to see test results showing adequate sterlization. You can take a
photograph of the results or if there is a way to print results from the machine you can do that and fax or email
them over to me. For the issue regarding the segregation of pharmaceutical waste, photographs as well as a
corrective action plan, or any kind of training documents given to staff will suffice. Basically we need evidence to
show how the facility is operating differently to fix the segregation issue. Let me know if this answers your
question.
Harprit S.
