Solid waste can be stored in a container previously <br /> used for medical waste, but only after the container has been properly decontaminated and <br /> all of the medical waste labels have been removed. Remove biohazard labels from the solid <br /> waste container immediately and submit evidence of compliance by 7/30/15. <br /> 2. 118307—Biohazard labels on the doors of the"soiled utility" room inside of the <br /> "Municipal Utility Room" (MUT) were not visible because the doors were open. A <br /> biohazard label was observed on the west entrance door of the backroom of the pharmacy, <br /> but not on the south entrance door. Intermediate storage areas shall be marked with the <br /> international biohazard symbol. These warning signs shall be readily legible from a <br /> distance of 5 feet. Ensure biohazard labels are visible at all times. <br /> -In"MUT", either close the doors to the"soiled utility"room, so that the biohazard labels <br /> are visible, or add labels that are visible when the doors are open. <br /> -In the pharmacy, either remove the label from the west entrance door of the back room or <br /> add a biohazard label on the south entrance door of the backroom. <br /> -Submit evidence of compliance by 7/30/15. <br /> 3. 118285—Sharps containers placed on the floor of the "Dialysis Clinic" were not labeled <br /> and/or labels were not visible. Sharps containers in the "Medication Rooms" of"134A" and <br /> "D413" were being used to store used batteries. Sharps containers must be used solely for <br /> the storage of biohazardous sharps waste. Sharps containers must be labeled with the words <br /> "sharps waste" or with the international biohazard symbol and the word"BIOHAZARD." <br /> Label containers immediately and submit evidence of compliance by 7/30/15. <br /> 4. 118280(c)—A biohazard bag was observed attached to the outside of a solid waste <br /> container in the "Dialysis Clinic." This bag was being used to store other biohazard bags <br /> and solid waste container bags. Biohazard bags must only be used for the storage of <br /> biohazardous waste. Biohazardous waste must be placed in a biohazard bag inside of a <br /> rigid container equipped with a tight fitting cover. Ensure biohazard bags are being used <br /> solely for the storage of biohazardous waste inside of a rigid container. This violation was <br /> corrected onsite. <br /> 2 <br />
