Pharmaceutical waste must be <br /> placed in a container or secondary container with labels on the lids and sides. Inspect <br /> pharmaceutical containers for defective lids. Ensure all pharmaceutical waste containers <br /> are equipped with functioning, closeable lids, and submit evidence of compliance by <br /> 7/30/15. <br /> 6. 118215(2)(B)—Autoclave thermometer has not been calibrated. Thermometers <br /> thermocouples, or other monitoring devices shall be checked for calibration annually. <br /> Records of calibration checks shall be maintained onsite for 2 years. Calibrate autoclave <br /> thermometer immediately and submit evidence of compliance by 7/30/15. <br /> 7. 118215(2)(D)—Monthly tests spore tests conducted to confirm attainment of sterilization <br /> conditions were unclear. Several tests did not pass or failed to specify test results. The <br /> dates on the spore test results did correspond with dates on the autoclave sterilization log. <br /> Conduct a spore test immediately. Autoclave must not be used until a negative spore test <br /> result is achieved. Provide training for employees who conduct spore testing. Inspect <br /> autoclave and spore testing equipment to correct the date variation. Submit evidence of <br /> compliance by 7/30/15. <br /> 8. 117967—Facility was unable to provide training records for autoclave operators. <br /> Autoclave operators must be trained upon hiring and annually thereafter. Records of <br /> training must be maintained onsite for 2 years. Immediately train all employees who <br /> operate the autoclave and submit evidence of compliance by 7/30/15. <br /> 3 <br />
