4 <br />LOCKOUT UT POLICY AND PROCEDURES . <br />living centers <br />FACILITY NAME <br />Purpose The purpose of this policy is to establish safe procedures for securing and <br />isolating sources of hazardous energy prior to the servicing or maintenance of <br />facility equipment or machinery. The policy is designed to comply with the <br />requirements of the OSHA standard, 29CFR1910.147, "Control of Hazardous <br />Energy" <br />Policy All facility employees and contract personnel will follow the procedures <br />outlined in this policy to assure all equipment is tagged out and/or locked out <br />prior to servicing or maintenance. All employees will be instructed in the <br />safety significance of the lockout/tagout procedure and their respective duties <br />with regard to the program. <br />Responsibilities The Executive Director is responsible for the overall implementation of this <br />program. <br />The Maintenance Supervisor is responsible for utilizing the procedures in this <br />policy when servicing or performing maintenance on facility equipment. <br />He/She shall develop specific procedures for each hard -wired piece of <br />equipment or equipment with multiple energy sources. These procedures will <br />be documented on the "Lockout/Tagout Equipment Isolation Procedure Form" <br />as shown in attachment "A". The Maintenance Supervisor will notify all <br />affected employees, prior to the start of service work, that the lockout/tagout <br />procedure is about to be implemented. The Maintenance Supervisor will <br />participate in the annual review of the lockout/tagout program, to include actual <br />review of all specific equipment procedures, as witnessed by another <br />"authorized" person. The annual review will be documented on the <br />"Lockout/Tagout Annual Review Checklist" shown in attachment "B". <br />Training as to their duties with respect to the lockout/tagout program, will be <br />provided for all employees and contract personnel during orientation and at <br />least annually thereafter by the Maintenance Supervisor. <br />Definitions Affected Employee - An employee whose job requires him/her to operate or <br />use a machine or equipment on which servicing or maintenance is being <br />performed. <br />Authotized Employee — A person who implements a lockout or tagout system <br />procedure on a machine or piece of equipment to perform servicing or <br />maintenance on that machine or equipment. <br />Golden Living LOCKOUTITAGOUT Page I of 10 <br />
