DUIES <br />LOCKOUT■ ■ ■ L'YY N PR YC <br /># <br />€Mrtg centers <br />Training All employees and contract personnel will receive training on their duties and <br />responsibilities with regard to the lockout/tagout program. This training will <br />be conducted and documented by the Maintenance Supervisor. All required <br />training documentation will be maintained in the Maintenance Supervisor's <br />office. <br />All "affected" employees will receive lockout/tagout training at the time of <br />hiring and at least annually thereafter. At a minimum, this training will <br />include: <br />• Significance of the lockout/tagout program. <br />• Hazards employed with failure to use lockout/tagout. <br />• Only authorized employees are allowed to service or perform <br />maintenance on equipment. <br />• Importance of not attempting to operate locked or tagged equipment. <br />• Never attempt to remove a lockout/tagout device. Only the person who <br />places the device may remove it. <br />• How to recognize lockout/tagout devices. <br />• The consequences of the unauthorized removal of a lockout/tagout <br />device. <br />All "authorized" employees will be given training prior to any initial <br />involvement in the lockout/tagout program. At a minimum, this training <br />will include: <br />• The requirements of CFR 1910.147 Control of Hazardous Energy. <br />• The types and magnitude of potential energy sources at the facility. <br />+ Limitations of the tagout system. <br />+ Lockout and/or tagout procedures for specific equipment. <br />• A procedure for testing equipment to assure energy has been isolated. <br />• Procedures for placing and removing lockout/tagout devices. <br />• Procedures for restoring energy safely. <br />• Requirements for notifying "affected" employees prior to implementing <br />and on completion of lockout/tagout procedures. <br />• Forms to be used to document specific procedures and annual review of <br />the program. <br />Retraining will be provided on an annual basis and whenever there is a change <br />in job assignment, or a change in equipment or processes that would create a <br />new hazard. <br />Golden Living LOCKO IT✓TAGOUT Page 2 of 10 <br />
