LOCKOUT TAG T POLICY AND PROCEDURES <br />living centers <br />Lockout/ Regardless of the equipment being serviced, there are several "generic" steps in <br />Tagout the lockout/tagout process, which are common to all. The following steps will <br />Procedures be included in all lockout/tagout procedures performed at this facility: <br />Step <br />Action <br />1 <br />Notify all "affected" employees that a lockout/tagout system is <br />going to be utilized. Document on the "Equipment Isolation Form" <br />which "affected" employees were notified. <br />2 <br />If the equipment is running, shut it down by normal stopping <br />procedures (hit stop button, flip toggle switch, etc.) <br />3 <br />Using the Equipment Isolation Procedure Form, locate the energy <br />isolating devices for the equipment and place the proper <br />lockout/tagout devices to isolate the energy source. Be sure there are <br />no "stored" energy sources. <br />Note: Stored energy (such as that in springs, elevated machine <br />members, rotating flywheels, hydraulic systems, and air, gas, steam <br />or water pressure, etc.) must be dissipated or restrained by methods <br />such as repositioning, blocking or bleeding down, etc. <br />4 <br />After verifying that no employees are exposed, verify the equipment <br />is de -energized by attempting to operate the equipment by normal <br />starting procedures. <br />CAUTION: After verifying that the equipment is de -energized, <br />return the operating controls to the "neutral" or "off" position <br />after the test. <br />5 <br />Perform the necessary service or repair. <br />6 <br />Inspect the area around the equipment to assure all tools and <br />materials are removed and the machine is "off' before re -energizing <br />the e ui ment. <br />7 <br />Notify all "affected" employees before removing the locks and <br />tags. <br />8 <br />Remove all locks and tags and place the energy isolating devices <br />back to the "on" position. <br />9 <br />Re-energize the equipment usin the normal start-up procedures <br />10 <br />Check equipment for normal operation. <br />When more than one individual is required to service a piece of equipment at <br />the same time, each individual shall have his/her own personal lockout device. <br />When the energy isolation device cannot accept more than one lock, a hasp <br />shall be used. <br />Golden Living LOCKOUTlTAGOUT Page 4 of 10 <br />
