LOCKOUT TT POLICY AND PROCEDURESr°:': g <br />living centers <br />Annual Each year an "authorized" person, other than the person involved in the <br />Review procedure being inspected, will conduct a review of the lockout/tagout program <br />and witness or evaluate the specific procedures for control of all energy <br />sources. This annual review will be documented using the form in attachment <br />"B". The "authorized" person may be a maintenance person from another <br />facility or a licensed electrician. <br />As an alternate method, the maintenance staff may perform and document the <br />annual review during the regular monthly/quarterly preventative maintenance <br />schedule for a specific piece of equipment. <br />Outside All outside contractors working in the facility will be briefed on the Golden <br />Contractors: Living lockout/tagout program and specific procedures for the job to be <br />accomplished. The contractor will be required to follow these procedures. If <br />the contractor elects to follow his/her own lockout/tagout procedures, then the <br />contractor must inform the appropriate facility personnel of the procedures to <br />be used. <br />Prior to any work the Maintenance Supervisor will complete and document <br />each step in the attached "Outside Contractor's Checklist" as shown in <br />attachment "C". The contractor must sign the "Contractor Acknowledgement" <br />as shown in attachment "D". All outside contractor documentation will be <br />maintained by the Maintenance Supervisor. <br />Enforcement: Failure to follow the procedures outlined in this policy for control of hazardous <br />energy may result in death or serious injury. As such, violations of this policy <br />may result in disciplinary action up to and including termination. <br />Golden Living LCiCKOUTITAGOUT Page 5 of 10 <br />
