Page 6
.,
D. In the (6), the double -bagged
infectious waste will be placed directly into a leak -proof,
rodent -proof, storage container with a tight fitting lid, provided
and maintained by Bingham. The storage container will be
identified with a numerical code designating the generating
institution and labeled with the words "Infectious waste', or the
international biohazard symbol and the word "Bi®hazard".
E. The storage containers will be located at the institution in
an area that is secured to deny access to unauthorized personnel.
This area will display prominent warning signs in English
"Caution --infectious waste storage area-- unauthorized persons
keep out", and in Spanish--"Cuidado -- zona de residuos
infectados -- prohibida la entrada a personas no autorizadas".
This area will be well ventilated and kept clean at all times.
F. The storage containers will be picked up by Bingham intact as
a unit, as often as required, but in any event, often enough so as
to avoid the putrification of the waste contained therein.
G. The volume of infectious waste to be transported and
incinerated or sterilized per week is estimated at (7) lbs.
H. Each new (8) will receive an in-depth
orientation in the procedure for the in-house collection,
transportation, and/or storage of infectious waste at this
institution prior to his/her first day on the job. Continual in-
service training will be supervised by the person in charge of
this area.
I. The local Director of Health will be promptly notified if this
Individual Plan for Off -Site Infectious Waste Disposal is either
discontinued or altered.
SIGNATURE
TITLE
DATE
