Section I 1 <br />Disposal of Medications, Syringes and Needles � Page 2 of 3 <br />5.4 -- <br />Disposal of Medications 1 09/10 <br />(California Specific) <br />• Medication name and strength <br />• Prescription number <br />• Quantity/amount disposed <br />• Date of disposition <br />• Signatures of the required witnesses <br />4. If a controlled medication is unused, refused by the resident or not given for any <br />reason, it cannot be returned to the container. Medication will be rendered unusable <br />prior to sealing the waste container and the disposal is documented on the <br />accountability record on the line representing that dose with the required signatures. <br />This same procedure applies to unused portions of single dose ampules and doses of <br />controlled substances wasted for any reason. <br />5. Medications not listed in Schedules II, III, IV and V (non -controlled medications) <br />shall be destroyed by the nursing care center in the presence of a pharmacist or nurse <br />and one other witness. Documentation of non -controlled medication may be <br />completed on a medication administration record (MAR), a medication disposition <br />log or form (or record provided for that purpose) and shall be retained as per federal <br />privacy and state regulations. Disposed medications are transferred to a container box <br />for release to a pharmaceutical waste contractor labeled "For Incineration Only". <br />(Medications may be left in the original vial or container for disposal). <br />a. A non -controlled medication disposition log or form shall be used for <br />documentation and shall be retained as per federal privacy and state regulations. <br />The log shall contain the following information: <br />• Resident's name <br />• Medication name and strength <br />• Prescription number, if applicable <br />• Quantity/amount disposed <br />• Date of disposition <br />• Signatures of the required witnesses <br />Nursing Care Center Pharmacy Policy & Procedure Manual - 02007 PharMerica Corp <br />
