infectious Waste Customers, <br />During a recent, inspection and monitoring period of <br />our facilities and work procedures, our insurance carrier <br />has requested that we no longer collect and dispose of in-- <br />fec.rious waste by means of 'bags only. We were advised that. <br />state law requires that, in addition to double plastic bags, <br />of the proper type, this material must also be placed into <br />a sealed ._ox. These }.-poxes must be properly identified with <br />the words "infectious waste" or with the international bio- <br />hazard symbol and the word "Bio -hazard". Therefore we must <br />immediately request that you as an infectious waste generator <br />adhere to this requirement. Our new pick up and disposal <br />rates. including providing the proper plastic bags and boxes <br />will be as follow: <br />A. Large Boxes(Appr'oximatley 6c u ft. in volume) <br />1 to 4 boxes <br />$25.00 ea box <br />5 to 10 boxes <br />$22.00 ea box <br />_ to 20 boxes <br />$20.00 ea box <br />B.
