SCOPE <br />This contract provides pharmaceutical reverse distribution and destruction services for the <br />State of California and participating local agencies. The contractor, Guaranteed Returns, <br />will provide the services listed in Exhibit A to the State. <br />This contract is accessed through participation in the Minnesota Multi -State Contracting <br />Alliance for Pharmacy ("MMCAP"). MMCAP's current contractor for pharmaceutical reverse <br />distribution and destruction services is Guaranteed Returns. <br />The contract term is through August 31, 2007. MMCAP anticipates awarding a successor <br />contract within 60 days. Agencies must join MMCAP as a participating member. Agencies <br />are instructed to complete Exhibit B, "Minnesota Multistate Contracting Alliance for <br />Pharmacy California Facility Membership Application". See Section 10, "Enrollment <br />Process for Agencies" below. As credits will be processed through the State's <br />pharmaceutical wholesale distributor, agencies that are not using the State's <br />pharmaceutical wholesale distributor must contact the DGS contract manager to determine <br />eligibility and enrollment. <br />The Contractor, Guaranteed Returns, will provide returned pharmaceutical goods <br />processing services, according to all applicable federal and state laws, to all current and <br />future participating. MMCAP members. The Contractor will provide reports and services as <br />described below in Section 7, "Contractor's Responsibilities." This contract processes <br />pharmaceutical, over the counter (OTC) and HBC (Health and Beauty Care) waste, <br />including non -creditable schedule merchandise CII -CV's and non -schedule items. This <br />contract does not process corrosive or flammable materials in excess of the amount defined <br />as exempted by UPS. <br />Controlled Substance Procedure. <br />The Contractor will follow the procedures set by MMCAP. The Contractor will separate the <br />Schedule III, IV and V to separate invoices. <br />
