Guaranteed Returns and <br />Califomia Department of Corrections (CDC) <br />SCOPE OF WORK <br />INTRODUCTION/SERVICES <br />Contract Number ICM04358 <br />Exhibit A <br />The Contractor agrees to furnish all labor and materials to provide removal, destruction or <br />credit of controlled (DEA Schedules 11 through IV) and non -controlled substances for California <br />Department of Corrections' (CDC) institutions identified in Attachment G as designated and <br />requested by the Institution's Health Care Manager (HCM)/Chief Medical Officer (CMO) or <br />designee. <br />2. LICENSES/PERMITS/CERTIFICATION REQUIREMENTS <br />Contractor must be a U.S. Drug Enforcement Administration (DEA) registrant' approved to <br />process and destroy controlled substances and must possess a current DEA distributor's <br />license and an Environmental Protection Agency Acknowledgement of Notification of <br />Hazardous Waste Activity Certificate. <br />Contractor shall possess and maintain throughout the term of this contract a current and valid <br />license to do business in the State of California and shall obtain at Contractor's expense any <br />and all necessary license(s), permit(s), and certificate(s) required by law for accomplishing any <br />work required in connection With this contract. <br />Contractors located within the State of California shall meet all terms and conditions for <br />operating a business in the city/county in which the business is headquartered. Contracts <br />which are corporations located within the State of California may submit a copy of its <br />incorporation documents/letter issued by the Secretary of State. Contractors located outside <br />the State of California shall meet all terms and conditions for operating a business iri'the state, <br />providence, or county in which it is headquartered and shall submit an affidavit to show that <br />the business is in good standing in that state; providence or county. <br />3. CONTRACTOR'S RESPONSIBILITIES <br />Contractor shall: <br />1. Provide staff to physically pull expired and unusable controlled and non -controlled substances <br />from the institution pharmacy shelves. <br />2. Document, on-site, all controlled and non -controlled substances to be shipped and provide a <br />written inventory to the institution's contact pharmacist listing all products removed from the <br />institutions pharmacy during the pickup. Prior to shipment, complete and provide to the <br />institution's contact pharmacist all required DEA documentation. <br />3. Properly package product for shipment to Contractor's facility for further processing. <br />Contractor shall provide appropriate packaging/shipping materials. Contractor shall be <br />responsible for proper packaging of products and shipment to the contractors facility. <br />Contractor shall assume title of products beginning with shipment to Contractor's facility. <br />Contractor shall be responsible for all shipping charges. <br />
