The report shall include the institution's <br />name, the reporting period, the dollar costs, and dollar credits directed to the prime vendor. If <br />direct credits or like -kind merchandise exchange is provided to the state or institution, these <br />amounts shall be separated from the State's prime vendor credits. <br />Maintain proper chain of custody requirements for all products shipped. <br />4. CREDIT FOR RETURNABLE ITEMS <br />Upon receipt at Contractor's facility, Contractor shall prepare returnable products for shipment <br />to manufacturers/wholesalers. Contractor shall be responsible for all shipping charges. The <br />State shall receive credit when applicable for products returned through the Contractor. <br />Contractor shall: <br />1. Apply the manufacturer's/wholesaler's credit to the institution's prime vendor account. <br />CDC will notify Contractor of any change in the prime vendor and/or account numbers: <br />and/or <br />2. Provide a reconciliation list of credits for all products returned to each contact pharmacist <br />within 30 days of return. <br />3. Provide replacement product in the exact amount of credit. <br />Contractor shall provide institution's contact pharmacist a listing of inventory shipped to each <br />manufacturer/wholesaler with the Calculated Return Value (CRV). <br />5. DESTRUCTION OF NON -RETURNABLE ITEMS <br />Contractor shall destroy all non -returnable products by high temperature incineration using an <br />EPA licensed waste processing facility. Contractor must provide State with a copy of the <br />facility's license prior to the commencement of services. Contractor shall provide contact <br />pharmacist with a Certificate of Destruction for non -returnable inventory that was processed as <br />waste. Payment for destruction of non -returnable items shall be contingent upon the receipt of <br />Certificate of Destruction by the institution's contact pharmacists. <br />Contractor shall provide a drug manifest list of all drugs to be destroyed to the contact <br />pharmacist within 30 days of destruction. <br />6. CDC'S RESPONSIBILITIES <br />CDC will provide: <br />1. Institution names, address, phone numbers and contact pharmacists; <br />2. Prime vendor account numbers for each institution; <br />3. Proper security clearance and escort for entry onto institution grounds; and <br />-2- <br />
