REQUEST FOR SERVICES <br />Contractor shall pick up and Process expired or unusable controlled and non -controlled <br />substances from each institution as determined by each institution, or upon request of the <br />institution. The institution and the Contractor shall determine a mutual date and time for pick <br />up. The institution will provide Contractor with not less than seven (7) calendar days advance <br />notice for . any month in which a pick up is not required. <br />If more frequent pick ups are required, the institution shall provide advance notice to the <br />Contractor not less than seven (7) calendar days prior to the requested pick up date. <br />8.
