Scope of Service -15- DISPOSAL SITES (cont____in_ued) 7J Attachment A Contract Number MLT97000 an alternate incinerator site shall be identified during the term of any resultant contract. Any change of disposal sites after the awarding of the contract must be approved, in writing, by the institution. All designated disposal sites for the institution medical waste must be approved by the State and local governments and have the required permits to operate as medical waste disposal sites. There shall be no pending regulatory or judicial actions against the sites. The Contractor shall be an individual or firm licensed to do business in the State of California and shall obtain at his/her expense all license(s) and permit(s) required by law for accomplishing any work required in connection with this contract. Licenses/permits not current shall be cause for termination of the contract. Contractor must be fully qualified and permitted and have sufficient qualified and permitted personnel available at all times during the term of the contract to properly perform the services required. Contractor shall provide CDC with copies of the following license(s) and permit(s): A. City/County Business License. B. Department of Health Services Medical Waste Permits. C. Food and Agriculture, Measurement Standard, Weigh Master License issued to the Hauler/Prime Contractor. D. Public Utilities Commission Operating Authority Permit for Property Transportation issued to the contractor/hauler (required when the disposal treatment is subcontracted). E Proof of Contractor's satisfactory rating on the California Highway Patrol BIT Program (Biennial Inspection of Terminal). A copy of the inspection report or a copy of the CHP MISTER (Management Informational Systems Terminal Evaluation Record) Form are acceptable as proof. F. Department of Toxic Substance Control (DTSC) Hazardous Waste Transporter Registration issued to the Hauler/Prime Contractor. Contractor shall carry a copy of the Hazardous Waste Transporter Registration certificate while transporting medical waste. Contractor shall bear the responsibility during transportation.
