Go <br />BFI Medical Waste Systems of CA., Inc. <br />Scope of Service -15- <br />DISPOSAL SITES (cont____in_ued) <br />7J <br />Attachment A <br />Contract Number MLT97000 <br />an alternate incinerator site shall be identified during the term of any resultant contract. <br />Any change of disposal sites after the awarding of the contract must be approved, in <br />writing, by the institution. <br />All designated disposal sites for the institution medical waste must be approved by the <br />State and local governments and have the required permits to operate as medical <br />waste disposal sites. There shall be no pending regulatory or judicial actions against <br />the sites. <br />The Contractor shall be an individual or firm licensed to do business in the State of <br />California and shall obtain at his/her expense all license(s) and permit(s) required by <br />law for accomplishing any work required in connection with this contract. <br />Licenses/permits not current shall be cause for termination of the contract. Contractor <br />must be fully qualified and permitted and have sufficient qualified and permitted <br />personnel available at all times during the term of the contract to properly perform the <br />services required. <br />Contractor shall provide CDC with copies of the following license(s) and permit(s): <br />A. City/County Business License. <br />B. Department of Health Services Medical Waste Permits. <br />C. Food and Agriculture, Measurement Standard, Weigh Master License issued to <br />the Hauler/Prime Contractor. <br />D. Public Utilities Commission Operating Authority Permit for Property <br />Transportation issued to the contractor/hauler (required when the disposal <br />treatment is subcontracted). <br />E Proof of Contractor's satisfactory rating on the California Highway Patrol BIT <br />Program (Biennial Inspection of Terminal). A copy of the inspection report or a <br />copy of the CHP MISTER (Management Informational Systems Terminal <br />Evaluation Record) Form are acceptable as proof. <br />F. Department of Toxic Substance Control (DTSC) Hazardous Waste Transporter <br />Registration issued to the Hauler/Prime Contractor. Contractor shall carry a <br />copy of the Hazardous Waste Transporter Registration certificate while <br />transporting medical waste. Contractor shall bear the responsibility during <br />transportation. <br />
