Sainz, M.D. <br />Deuel Vocational Institution <br />PO Box 378001 <br />Tracy CA 95378 <br />RE: Medical Waste Inspection Report <br />Dr. Sainz, <br />On December 31, 1996, I inpsected DVI for compliance with the Medical Waste Management <br />Act and found the violations noted on the inpsection report (copy enclosed) I left with Don <br />Wilcoxen on the day of the inpspection. To this day I have not received any written <br />documentation that the violations noted have been corrected. If the violations have been <br />corrected please submit documentation to confirm this by 3/7/97. If the violations have not <br />been corrected please explain why this has not occurred.
