Provide a completed medical waste tracking document at the time of each pick-up, <br />pursuant to HSC, Chapter 6, Section 118040. The document shall be properly executed, <br />signed and dated by the Supervising Registered Nurse II or designee and the waste hauler <br />at each of the institution locations. Only the Supervising Registered Nurse 11 or <br />Correctional Health Services Administrator I or designee shall review and sign the <br />original manifest (for billing authorization) prior to shipment of medical waste. <br />Within fifteen (15) calendar- days after medical waste pick up, a copy of the manifest with <br />the disposal site information section completed shall be furnished to Cynthia Fedrick, <br />Correctional Health Services Administrator I, or designee. Invoices will be processed <br />upon receipt by the Correctional Health Services Administrator I or designee to include, a <br />copy of the treatment, storage and disposal (TSD) facility manifest, establishing <br />acceptance of the waste at the disposal site, and the Certificate of Disposal from the <br />ultimate disposal facility. <br />Contractor shall provide an official scale, approved by the State Department of Food and <br />Agriculture Measurement Standards Division, to weigh medical waste. Contractor is <br />responsible for weighing all medical waste at the institution prior to transport. Contractor <br />shall report the total pounds picked up to the institution liaison prior to transport. <br />Rev. 06/07 <br />H. Pharmaceutical Waste: <br />1. Partially used/residual prescription or over the counter medication, such as tablets, <br />capsules, vials with residual (pourable) liquids, powders, eye drops, creams/lotions, <br />partial tablets and suppositories. <br />2. Residual or wasted narcotics, other controlled substances and/or dangerous drugs. <br />3. Used patches. <br />4. Syringes, tubex, carpujects (without sharps), with residual medication. <br />5. IV bags and tubing with residual medication. <br />Unopened/unused or expired medications are returned to the Pharmacy in a timely manner. <br />Pharmaceutical waste is disposed of in white containers, labeled "Incineration Only". <br />Pharmaceutical waste containers will be secured in areas that are not accessible to the inmate. <br />Medical waste and sharps are never disposed of in the pharmaceutical waste container. Each <br />container must be clearly labeled with the current date (when put into service) and the expiration <br />date. When the waste container becomes 114 full; the following will occur: <br />The designated medical staff (RN/LVN) assigned to the area will contact the <br />pharmacy to arrange for pick-up and for proper handling. <br />The Pharmacist in Charge (PIC) or his designee will contact the pharmaceutical <br />contract provider Guaranteed Returns at 1-800-473-2138 to arrange for a pick- <br />up. <br />When needed a representative of Guaranteed Returns will assist with the <br />preparation package and shipping. <br />
