Pharmaceutical and medical waste shall be segregated and contained pursuant to this section. The container shall be <br /> properly labeled HSC 118275 The pharmaceutical waste container was not labeled with the date that it was put in use. <br /> Submit evidence of correction to the EHD by 2/1/19. <br /> 25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair <br /> and appropriately labeled. <br /> - Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tightfitting <br /> cover, and be kept clean and in good repair. Container shall be labeled with the words <br /> "Biohazardous Waste"or"BIOHAZARD"on the lid and on the sides so as to be visible from any lateral <br /> direction. HSC 118280(c) The biohazardous waste containers on the floor are missing stickers on the sides. Submit <br /> evidence of correction to the EHD by 2/1/19. <br /> All corrections are due by 2/1 /19. <br /> Notes: <br /> 1) Stericycle is the licensed medical waste hauler. The facility has waste picked-up twice per week. <br /> 2) Pharmaceutical waste is picked-up at least once per month. <br /> 3) The Medical Waste Management Plan is available onsite, and annual training records are available for the 12 employees <br /> which come in contact with medical waste. <br /> 4) The biohazardous waste bags meet both the ASTM D1709 and D1922 standards. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
