HSC 118280 -Biohazard bags shall be containerized and stored pursuant to this section. <br /> OBSERVATIONS: <br /> Two biohazard containers near the nurses' station were not properly labeled (see PHOTO 1 and PHOTO 2). <br /> CORRECTIVE ACTIONS: <br /> Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be kept <br /> clean and in good repair. Container shall be labeled with the words"Biohazardous Waste" or"BIOHAZARD" on the lid and on <br /> the sides so as to be visible from any lateral direction. Properly label the biohazard waste containers and provide photographic <br /> evidence of correction to the EHD. <br /> Correct all violations and provide evidence of correction to the EHD <br /> by October 20, 2026. <br /> Notes: <br /> 1. Inspection report emailed to Jamelle Johnson, Facility Administrator, at Jamelle.iohnson(a)davita.com. <br /> 2. Medical waste is picked up by Stericycle (licensed waste hauler)three times a week. <br /> 3. Biohazardous waste, sharps waste and pharmaceutical waste is generated by the facility. <br /> 4. Ensure biohazard labels are on all four sides and the lid of biohazard containers. <br /> 5. Medical Waste Management Plan was updated on September 30, 2025 and a copy was provided to the EHD. <br /> 6. Treatment and tracking records were reviewed. <br /> 7. Biohazard bags are ASTM D-1709 and ASTM D-1922 certified. <br /> 8. Facility has begun utilizing biohazard containers for biohazard waste streams and for solid waste streams. Ensure all waste is <br /> inside of the container(see PHOTO 1). <br /> Reinspecion on/about: A reinspection fee of$179 per hour may be charged. Page 2 of 2 <br /> EH-2/2023 <br />
