Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair <br /> and appropriately labeled. <br /> - Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tightfitting <br /> cover, and be kept clean and in good repair. Container shall be labeled with the words <br /> "Biohazardous Waste" or"fib"on the lid and on the sides so as to be visible from any lateral <br /> direction. HSC 118280(c) The containers must be labeled on the lid and all 4 sides. <br /> Notes: <br /> 1) The facility uses Stericycle as a licensed Medical Waste Hauler. Medical waste is picked-up from the facility twice per <br /> week. <br /> 2) Tracking records are available for the last two years. The weights for the shipments are available. No records of final <br /> disposal are available. <br /> 3) The biohazardous waste bags meet both the ASTM 131922 and D1709 standards. <br /> 4) The facility has 17 employees.
