III. ACCUMULATION/STORAGE SITE <br /> • All regulated medical waste are collected by Stericycle and maintained in an enclosed and designated <br /> accumulation area located right next to grossing room#2.This area is marked with the following warning <br /> sign in both English and in Spanish: <br /> " CAUTION—BIOHAZARDOUS WASTE STORAGE AREA- <br /> UNAUTHORIZED PERSONS KEEP OUT" <br /> "CIUDADO—ZONA DE RESID UOS—BIOLOGICOS PELEIGROSOS- <br /> PROHIBIDA LA ENTR, DAA PERSONAS NO A UTORIZADAS" <br /> IV. DISPOSAL, COLLECTION AND TRANSPORT: <br /> • Stericyle collects,and transports the medical waste from our facility to an approved site for treatment and <br /> disposal.The waste are collected weekly every Wednesday. <br /> • All waste will be accompanied by a tracking document which will include the generator's and the <br /> transporter's name,addresses, customer#,description of waste,and other pertinent information.The <br /> document will be signed by both Delta Pathology and Stericycle. LIQUID WASTE MANAGEMENT: <br /> • Clean Harbors collects, and transports chemical liquid wastes(solvents)from our facility to an approved site <br /> for treatment and disposal.The waste are collected in a black 55 gallon drum which is marked and labeled <br /> with the hazardous waste label and required information filled out.(generator's ID,description of contents, <br /> accumulation start date,etc.). <br /> • The waste drum is maintained in a well ventilated area until collection and transport by Clean Harbors.This <br /> area is marked with appropriate warning labels and PPEs and universal precautions are observed when <br /> handling this type of waste. <br /> • After formalin,xylene,methanol, alcohol,and staining solutions are drained and collected separately,the <br /> liquids are transferred and funneled in the appropriate 55 gallon liquid waste drum.A log is kept on the types <br /> and the amount of liquid waste disposed. Clean Harbors will be notified for collection when the barrel reach <br /> %full. <br /> • Any items such as paper,rags and packaging materials that are contaminated will be placed in a separate <br /> black 55 gallon waste drum properly marked with hazmat info and labeled with contents description(i.e. <br /> soaked paper,rags,etc.). <br /> • The drums will be retained for up to 90 days or when full. (whichever comes first).Per Clean Harbors <br /> transport personnel, 180 days are allowed in some cases.Verify local jurisdiction policies for complete <br /> protocol. <br /> • The manifest will be signed by both Delta Pathology and Clean Harbors transport personnel.A copy will be <br /> retained in Delta Pathology and another copy will be mailed within 30 days of collection to: <br /> Department of Toxic Substances Control <br /> P O Box 400 <br /> Sacramento,CA 95812 <br />
