ENDOSCOPY CENTER OF LODI <br /> Title: Hazardous Materials and Regulated POLICY: EOC 300 <br /> Medical Waste Management Plan Page 6 of 6 <br /> State: EFFECTIVE: 4/19/2012 <br /> CMS: 416.42, 416.44 a 3 , 416.49(b)(2) 416.51 REVISED: <br /> TJC: EC.02.01.01.01 <br /> AAAHC: 7-I,J 8-M <br /> All bio-hazardous waste shall be stored, handled or transported in containers that are <br /> leak resistant, have tight fitting covers and are kept clean and in good repair. <br /> This area shall be marked with warning signs saying in English and Spanish "CAUTION <br /> — BIOHAZARDOUS WASTE STORAGE AREA — UNAUTHORIZED PERSONNEL <br /> KEEP OUT." <br /> Biohazard bags and filled sharps containers shall not be stored for more than seven (7) <br /> days at a temperature above 32 degrees Fahrenheit. This seven (7) day period begins <br /> when any waste has been placed in the bag or container. <br /> See 118280 D1 (B) for 30-day allowance. <br /> Medical Waste Treatment and Disposal Method <br /> All Medical Waste will be picked up one time weekly by "Stericycle" Hauler and <br /> transported to the off-site treatment: <br /> Stericycle, Medical Waste Hauler <br /> 11875 White Rock Rd. <br /> Rancho Cordova, CA 95742 <br /> (866) 783-7422 <br /> Records of Medical Waste transported to "Stericycle" will be kept in the Nurse <br /> Manager's Office and retained for a minimum of three (3) years. <br /> Emergency Action Plan <br /> While it is highly unlikely that the company would experience a shutdown of operations, <br /> contingencies include use of an alternate hauler or retention of the medical waste for <br /> not more than seven (7) days from the point of generation above 0 degrees Centigrade <br /> (32 degrees Fahrenheit). <br /> Another contingency for less well-defined emergency situations would be to contact the <br /> Local Enforcement Agency (Department of Health Services, Medical Waste Program) <br /> for guidance at (916) 875-8469. <br /> Certification: <br /> The information included in the Medical Waste Management Plan is complete and <br /> accurate to the best of my knowledge. <br /> Administrator <br /> Regent Surgical Health-1/1/12 <br />
