• Documentation of the route of exposure and circumstances under which <br />exposure occurred. <br />• The exposed employee's relevant medical records including vaccination <br />status. <br />6.4.4. Healthcare Professional written opinion <br />• A copy of the evaluating Healthcare Professional's written opinion shall be <br />obtained and provided to the employer within 15 days of the completion of <br />the evaluation. The written opinion shall be limited to the following <br />information: <br />• Whether HB vaccination is indicated for the employee and if the employee <br />has received such vaccination. <br />• That the employee has been informed of the results of the evaluation. <br />• That the employee has been told about any medical conditions resulting <br />form exposure to blood or other potentially infectious materials which <br />require further evaluation or treatment. <br />• All other findings or diagnosis will remain confidential and will not be <br />included in the written report. <br />6.4.5. Procedure Evaluation — The procedures for evaluation of an exposure incident <br />are as follows: <br />• Investigate the circumstances surrounding the incident. <br />• Evaluate the policies in place at the time when the incident occurred. <br />• List the engineering controls in place when the incident occurred. <br />• List the work practices and protective equipment used at the time of <br />incident. <br />• Determine the cause(s) of failure(s) that allowed the incident to occur. <br />• Identify corrective actions to preclude a repeat of a similar incident. <br />6.4.6. Record Keeping <br />• Medical Records — The company must keep all medical records for <br />employees with occupational exposure to infectious materials for the <br />duration of their employment plus 30 years. All records are confidential. <br />Medical files shall be established for each employee by name and Social <br />Security Number and will include the following: <br />• A copy of the employee's Hepatitis B vaccine status including dates of <br />vaccination or copy of the signed declination. <br />• The actual incidents when exposure or potential exposure occurred. Include <br />time and date of incident, how it occurred, and the results of tests performed <br />on the employee. <br />• Copy of the results from a medical examination following an exposure <br />incident. <br />• A copy of the healthcare professional's written opinion as required. <br />• A copy of the information provided to the healthcare professional as <br />required. <br />6.4.7. Training Records — All training records must be maintained for three years <br />and include the following information: <br />• Dates of the training sessions. <br />12.09 -23- <br />
