Field Operations Policy and Procedures <br /> 3. Decedent Removal <br /> 3.1 First Call <br /> Policy. First calls received as notification of a death must be conducted in a manner that <br /> facilitates the gathering of important decedent information while acting in consideration of the <br /> needs of the Authorized Representative. Refer to the Dignity Memorial Promise and the Dicey <br /> Honors A.C.E. Program for additional information. <br /> Procedure. <br /> When a Company representative receives a first call: <br /> 1. Complete an Initial Notification Form(FIN-CS051). <br /> Note: For locations that have implemented Carepoint, the First Call screen in Carepoint <br /> is an acceptable alternative to the Initial Notification Form (FIN-CS051). <br /> 2. Read the decedent's information on the Initial Notification Form (FIN-CS051) back to <br /> the caller for verification of each item. <br /> 3. Inform the caller that a Company representative will be dispatched to conduct the <br /> removal and the client family will be contacted by a funeral director. <br /> 4. If the decedent is located at home and medical professionals are not present, inform the <br /> caller to contact local authorities prior to removal for release. <br /> 5. Assign the decedent case ID number, if applicable. <br /> 6. Contact the PCC or removal personnel immediately after completing the first call and <br /> forward a completed and legible copy of the Initial Notification Form (FIN-CS051). See <br /> Decedent Transfer for removal transfer policy and procedures. <br /> 3.2 Identification Confirmation at Place of Removal <br /> Policy. Upon arrival at the place of removal (i.e. place of death, home, hospital, medical <br /> examiner's office), each Company representative performing the removal must confirm the <br /> decedent's identity with the releasing agent. <br /> Procedure. <br /> 1. Compare all decedent information on the Initial Notification Form (FIN-CS051) to the <br /> institution's paperwork and identification tag (if applicable) and resolve any <br /> inconsistencies before removing the decedent. If decedent information is inconsistent or <br /> if the institution provides no paperwork or identification tag, stop the process until the <br /> decedent's identity can be conclusively confirmed.
