The Authorized Representative signs an Acknowledgment and Release of Liability <br /> Regarding Mortuary Services Form. The Legal Department may require additional <br /> documentation to proceed with the use of a non-Company contractor. <br /> Inspection of third-party facilities: <br /> 1. Use the Mortuary Services Inspection Checklist, provided by the Legal Department, to <br /> complete the inspection. <br /> 2. Retain a copy of the Mortuary Services Inspection Checklist at the market management <br /> office with a copy sent to the Legal Department. <br /> 2.8 Services for Non-Company Affiliated Locations <br /> Policy. Non-Company affiliated locations who have services performed or provided by <br /> Company locations must follow Company guidelines, policies and procedures, including the <br /> completion of all required forms and agreements, and must submit to at least a bi-annual <br /> inspection. <br /> Additionally, the non-Company affiliated location associates must be thoroughly trained in all <br /> processes, based on the services provided, detailed within this document prior to the Company <br /> location conducting said services (i.e. removals, identification confirmation or mortuary <br /> services). See the Cremation Manual for information on cremation services provided for non- <br /> Company affiliated locations. <br /> Procedure. <br /> When services are to be performed or provided for non-Company affiliated locations: <br /> 1.
