Affix the completed white identification band to the decedent's left ankle. <br /> a. If the decedent's left ankle is not available for placement of the white identification <br /> band,place the white identification band on another extremity in the following order: <br /> i. Right ankle <br /> ii. Left wrist <br /> iii. Right wrist <br /> 5. In the event the decedent is contained in a disaster pouch or wrapped in plastic due to <br /> communicable disease or decomposition, securely attach the white identification band to <br /> the outside of the pouch or wrappings. <br /> Notes: <br /> • Only locations with additional state requirements for information on identification <br /> bands will be allowed to use toe tags in place of white identification bands. Use of <br /> toe tags must be approved by the Operations Support Department and must be <br /> attached accordingly with a cable lock strap. <br /> • Creation and use of colored or duplicate bands/tags are prohibited for use of <br /> identification or any other purpose unless currently stated in policy and procedure <br /> (i.e. an identification band or an identification label attached to the outside of an <br /> urn). <br /> • In cremation cases, the white identification band will be cremated with the <br /> decedent as the metal identification disk becomes the primary identification tag <br /> (see Metal Identification Disk, Cremation Manual).
