Field Operations Policy and Procedures <br /> 2. If personal effects are to be cremated with the decedent, place them in the cremation <br /> container with the decedent. <br /> 3. If personal effects are to be placed with cremated remains after the cremation process: <br /> a. Place the personal effects in a clear, sealable bag with a label showing the decedent's <br /> full legal name (LAST, First Middle), date of birth, date of death, funeral home name <br /> and number, and case ID number(if applicable). <br /> b. Place those personal effects with the decedent's accompanying paperwork or in a <br /> secure area while the cremation is taking place. <br /> c. After the cremation, place the personal effects in the minimum acceptable container <br /> or urn with the cremated remains. <br /> 4. The Crematory Operator makes a final check for personal effects prior to starting the <br /> cremation. If any undocumented personal effects are found or if any inventoried personal <br /> effects are missing, stop the process and notify the funeral home and have them contact <br /> the family for instructions on the disposition of those items. <br /> Note: When state, provincial or local law prohibits (or a non-SCI crematory declines)the <br /> opening of the cremation container by a Company representative, the Company <br /> representative will not perform a personal effect verification, confirmation of personal <br /> effects must occur prior to arriving at the crematory. <br /> Releasing personal effects to the family: <br /> I. The funeral home representative fills out the Acknowledgement of Return of Personal <br /> Effects Form (FIN-CS109), detailing items to be returned to the decedent's Designated <br /> Representative. <br /> 2. The person receiving the items signs as the "Designated Representative" in the Personal <br /> Effects Released To section of the form. <br /> 3. The Company representative signs in the Personal Effects Released By section of the <br /> form. <br /> Destroying personal effects: <br /> 1.
