Field Operations Policy and Procedures <br /> 5.8 Collecting Fingerprints <br /> Policy. Collection of a decedent's fingerprints or footprints ("samples") is allowed only for <br /> purposes of creating keepsake articles (i.e. jewelry, card) and must be authorized by the <br /> Authorized Representative. After collection, such samples will be temporarily secured at the <br /> funeral home in the decedent's file and will be released for no other purpose. <br /> Note: Company representatives are not trained nor authorized to collect samples (prints, locks <br /> of hair, saliva, etc.) for DNA purposes. Families must contact a third party source for such <br /> requests and arrangements will be made to have the decedent available (with the Authorized <br /> Representative's permission) to the third party source for collection of samples. Contact the <br /> Legal Department for any questions. <br /> Procedure. <br /> 1. Explain to the Authorized Representative that the collection of one or more samples from <br /> the decedent can be done but only for the purpose of creating keepsake articles such as <br /> jewelry or commemorative cards. <br /> 2. Explain that one or more of the following conditions may apply to collection of samples: <br /> a. In some cases, the condition of the decedent may make it difficult or impossible to <br /> collect such samples. <br /> b. Tissue-enhancing chemicals may be used to facilitate the collection of samples. <br /> 3. Complete the Authorization to Collect Fingerprint Form (FIN-CS099) and have it signed <br /> by the Authorized Representative. <br /> a. If the Authorized Representative is not sure if they want to purchase a keepsake for <br /> samples at this time, check mark the box "Collect Sample from Decedent and <br /> temporarily hold for later use by Vendor" and check the box corresponding to the <br /> sample to be collected and have the form signed by the Authorized Representative. <br /> 4. Forward the Authorization to Collect Fingerprint Form (FIN-CS099) to the appropriate <br /> personnel at the funeral home or PCC for collection of the samples. <br /> 5. Collect the authorized samples from the decedent. <br /> 6. After collection, make a copy of the samples. <br /> 7. Transfer custody of the original samples collected only to the authorized vendor and <br /> forward the copy of said samples to the funeral home for temporary storage. <br /> 8.
