Minimal preparation includes washing the hair and body, setting features, suturing and <br />aspiration as well as casketing the decedent. These services may also be declined by the <br />Authorized Representative by checking the decline box on the form. <br />Authorization to Embalm <br />The Authorization to Embalm Form (FIN-CS007) documents permission received from the <br />Authorized Representative to provide specific preparations—particularly embalming—for the <br />decedent. If verbal approval for embalming can be attained during the initial call, this form must <br />be completed during the arrangements conference. If the funeral home uses a PCC, the form <br />must be forwarded to the PCC within one hour of the conclusion of the arrangements conference. <br />Authorization to Collect Fingerprint <br />The Authorization to Collect Fingerprint Form (FIN-CS099) allows the Authorized <br />Representative to authorize or decline collection of fingerprints or infant footprints from the <br />decedent for use in keepsake articles such as jewelry or cards. <br />Ceremonial Casket Acknowledgement <br />The Ceremonial Casket Acknowledgement Form (FIN-CS070) is to be completed any time an <br />Authorized Representative requests the use of a ceremonial casket. <br />Chain of Custody and Identification Confirmation <br />The Chain of Custody and Identification Confirmation Form (FIN-CS004) chronologically <br />documents the physical location and chain of custody of the decedent. It acknowledges that <br />Company representatives accepting custody of the decedent have compared and matched the <br />name on all accompanying paperwork to the white identification band for accuracy and <br />confirmation of identity. For each case involving cremation, it acknowledges that Company <br />representatives accepting custody of the decedent compared and matched the name on all <br />paperwork with the name on the label of the minimum acceptable container and the number on <br />the label to the number on the metal identification disk. <br />Where state, provincial or local law prohibits (or a non -Company crematory declines) the <br />opening of the cremation container by a Company representative, the Company representative <br />must match the name on all accompanying paperwork to the name written on the cremation <br />container or the casket identification tag. <br />Case Preparation Report <br />The Case Preparation Report (FIN-CS019) must be filled out after embalming and/or preparation <br />processes are finished.
