You are required to apply to <br /> EHD for a permit to operate a NTNC water system. To proceed with this process, fill out the enclosed <br /> forms and return them to the attention of Unit IV at EHD with the required permitting fees within 30 days <br /> of the date of this letter, as detailed below. <br /> �. Complete the Public Water Supply Permit Application form. <br /> ,,-'2. Complete and sign the blue and green Master File Record forms. <br /> ,e"3'. Complete the Emergency Notification Plan. <br /> w,-4. Provide a copy of a valid driver's license of the person signing the application. <br /> 5. Provide a Tax ID Number or the Social Security Number of the person signing the application. <br /> 6. Submit a check in the amount of$310 for the NTNC Permit Application fee. Please make the <br /> check payable to EHD. <br /> 7. Submit a check in the amount of$363 for the annual operating fee Please make the check <br /> payable to EHD separate from the Permit Application fee. <br /> 8. According to the California Health and Safety Code section 116540, "no public water system that <br /> was not in existence on January 1, 1998, shall be granted a permit unless the system demonstrates <br /> to the Department that the water supplier possesses adequate financial,managerial, and technical <br /> capacity to assure the delivery of pure,wholesome, and potable drinking water. This section shall <br /> also apply to any change of ownership of a public water system that occurs after January1, <br /> 1998". Complete and submit the enclosed TMF Capacity Assessment Form to this office for <br /> review <br /> A new water system must submit the following information to EHD in order to demonstrate that the <br /> system meets the standards of the Safe Drinking Water Act and requirements of San Joaquin County Well <br /> Standards as a NTNC: <br /> 1. Provide EHD a detailed schematic of the property showing the location for the new well,pressure <br /> tank,backflow prevention assemblies,all water lines(i.e. irrigation, fire, and domestic), septic <br /> system, 100%replacement septic area, and potential sources of contamination. Include the <br /> distances between these locations and the well. <br />
