Daily log book of information concerning the site including fires, <br />landslides, earthquake damage, unusual and sudden settlement, injury <br />and property damage accidents, explosions, receipt of and rejection of <br />unacceptable wastes, flooding and other unusual occurrences. <br />4-4. .Copy of written notification to the enforcement agency, local health <br />agency, and fire authority of the names, addresses, and telephone <br />numbers of the responsible County and site operation personnel. <br />2-.5.
