5.0 Hazard Identification and Control <br /> In addition to the Task-Specific JSAs, Section 5.1 lists the general procedures and practices common to <br /> most projects. For additional information, refer to CB&I required health and safety procedures or consult <br /> with your health and safety professional. A copy of all required CB&I Health and Safety Procedures are <br /> maintained in the CB&I office managing this project. CB&I Health and Safety Procedures shall be <br /> followed at all times. <br /> 5.1 General Hazards and Controls <br /> 5.1.1 General <br /> • Observe the following general procedures and practices: <br /> • Legible and understandable precautionary labels shall be affixed prominently to containers of <br /> potentially contaminated soil,water,and clothing. <br /> • An emergency eyewash unit shall be located immediately adjacent to employees who handle <br /> hazardous or corrosive materials, including decontamination fluids. All operations involving the <br /> potential for eye injury, splash, etc., must have approved eyewash units locally available capable of <br /> delivering at least 0.4 gallons per minute for at least 15 minutes. <br /> • All on-site activities will be conducted during daylight hours. If work after dusk becomes necessary <br /> due to an emergency, adequate lighting must be provided and notification of such activity made to <br /> the location contact. <br /> • Hazardous work, such as handling hazardous materials and heavy loads, and equipment operation, <br /> etc., should not be conducted during severe storms. <br /> • All temporary electrical power must have a ground fault circuit interrupter (GFCI) as part of its <br /> circuit if the circuit is not part of permanent wiring. All equipment must be suitable and approved <br /> for the class of hazard present. <br /> 5.1.2 Incident Reporting <br /> Observe the following incident reporting procedures and practices: <br /> • All occupational injuries/illnesses, vehicle accidents, and near miss incidents must be reported <br /> promptly to the PM and HSR and investigated as per CMS-710- 05-PR-02200 (See Appendix C for <br /> incident forms). <br /> • Immediately notify the PM and HSR when an incident occurs. <br /> • All OSHA recordable injuries/illnesses and chargeable vehicle accidents must be reviewed by an <br /> Accident Review Board report. <br /> 5.1.3 Daily Safety Meetings <br /> Daily safety meetings make accident prevention a top priority for everyone and make them aware of <br /> important accident prevention techniques. Site personnel must follow CMS-710-05-PR-01900 when <br /> conducting safety meetings. Observe the following daily safety meetings procedures and practices: <br /> • Daily safety meetings will be held each morning prior to site activities utilizing <br /> • Direct CB&I subcontractors are required to attend all tailgate meetings. <br /> • The tailgate meeting form in Appendix A will be used to document the meeting. <br /> 5-1 <br />
