5.1.4 Safety Inspections <br /> Site HSE Inspections shall be conducted as per CMS-710-05-PR-01400. Observe the following safety <br /> inspection procedures and practices: <br /> Daily Requirements <br /> • All Site Supervisors shall conduct daily HSE inspections and record their findings on the Daily HSE <br /> Site Inspection Form (CMS 710-05-FM-01401) (Appendix A). They shall also ensure that unsafe <br /> acts/conditions are corrected immediately. <br /> Weekly Requirements <br /> • Each site supervisor shall lead a formal written HSE inspection of his or her work area each week. <br /> The inspection team shall consist of the site supervisor and/or general foremen and the HSE Leader <br /> of the Week. <br /> • The Project HSE Inspection Form (CMS 710-05-FM-01402) (Appendix A) shall be used to <br /> document this inspection. <br /> Monthly Project Managers Inspection On a monthly basis, the Project Manager shall conduct an <br /> additional compliance inspection using the Project HSE Inspection Form (CMS-710-05-FM-01402) to <br /> document this inspection. The results shall be discussed with the Site Supervisor, work crew and a copy <br /> shall be forwarded to the Area HSE Manager. <br /> 5.1.5 Slip/Trip/Fall <br /> Observe the following procedures and practices to prevent slips/trips/fall: <br /> • Inspect each work area for slip/trip/fall potential prior to each work task. <br /> • Slip/trip/fall hazards identified must be communicated to all personnel. Hazards identified shall be <br /> corrected or labeled with warning signs to be avoided. <br /> • All personnel must be aware of their surroundings and maintain constant communication with each <br /> other at all times. <br /> 5.1.6 Underground Utility Line Contact <br /> Observe CB&I Procedure CMS-710-02-PR-01600. In addition, observe the following <br /> underground/utility line contact procedures and practices: <br /> • Follow applicable work instructions regarding training requirements, completion of utility mark-out <br /> documentation and pre-drilling/boring/direct push check list and variance request. <br /> • Contact client or facility owner to have utility lines marked prior to excavation/trenching or drilling. <br /> • Refer to site drawings or customer interviews if on private property for utility locations. <br /> • Hand dig, probe, post hole dig or air knife to 5 feet down and 5 feet to each side of utility marker to <br /> avoid breaking utility lines. <br /> • Prior to all intrusive activities (e.g., drilling, excavating), outside the footprint of the landfill, a <br /> locator line services will be contacted to mark underground lines, as-built drawings will be reviewed <br /> for presence of utilities in the work area and either hand digging probing, air knife to 5 feet below <br /> ground surface must be conducted. If the local geology be prone to refusal or should there be any <br /> other reason the boring location cannot be cleared to a minimum of 5' bgs, then geophysical <br /> methods (ground penetrating radar, magnetometer, etc.)Drilling and excavating activities within the <br /> 5-2 <br />
