First Aid procedures are on the following pages.) <br />3. Dial 911 if an ambulance is needed. <br />4. When you call for emergency help, inform them you are calling from Forward, Inc. <br />Landfill. <br />5. State where the emergency situation is, with the cross streets: <br />9999 S. Austin Road, Manteca, California, between Arch Road and French Camp <br />Road, north of Little John Creek. <br />6. State the telephone number you are calling from: (209) 982-4298. <br />7. State your first and last name. <br />8. State what happened - (heart attack, auto accident, fall, etc.) <br />9. State victim(s) condition - (unconscious, going into shock, alert, etc.) <br />10. State what is being done for the victim(s). <br />11. DON'T HANG UP FIRST. Let 911 hang up first. Before hanging up wait to hear <br />the dial tone. <br />12. If the injured or ill employee can be transported: <br />* Call Dameron Hospital Occupational Services 461-3194 and notify <br />them of the injury or illness. <br />* Take the injured employee to Dameron Hospital, Emergency Room Entrance, <br />525 West Acacia Street, Stockon, California 95203. <br />* Take an "Authorization For Medical Services" form to Dameron Hospital with <br />the injured or ill employee. <br />
