13. Report the incident to the nearest CAL/OSHA district office within 24 hours. <br />* If an injured employee is hospitalized for more than 24 hours for more than <br />routine observation. <br />* If an employee is fatally injured use the Preliminary Report of Industrial Accident <br />form to report the incident. Forward, Inc. must provide an "Employee Claim Form for Worker's Compensation <br />Benefit's" to the employee within 24 hours of learning of the injury or illness. <br />Follow Up <br />Investigate the accident. <br />* Find out what happened. <br />* Who saw what happened. <br />* How could the accident have been prevented. <br />* Take pictures if the situation warrants. <br />2. Use the "Accident Injury and Illness Investigation Form" or the "Supplementary <br />Record of Occupational Injuries and Illness" available from CAL/OSHA. <br />3. The "Employer's First Report of Occupational Injury and Illness must be completed <br />when an employee suffers and occupational injury or illness if: <br />* The occupational injury/illness results in lost time (defined as an absence from <br />work for a full day or shift beyond the date of the injury or illness). <br />* The occupational injury or illness requires medical treatment beyond first aid. <br />Note: You do not need to file an "Employer's First" if the occupational injury or illness <br />results only in first aid treatment or there is no lost time. The "Employer's First" must be <br />filed with your Worker's Compensation insurance carrier within five days, or the Division <br />of Labor Statistics if you are self-insured. <br />
