6.0-A PROFESSIONAL CERTIFICATION OF ACCURACY AMENDMENT NO. 4 0 11 Current regulations require that a registered civil engineer or a certified engineering geologist prepare and certify the accuracy of closure plans for all Class II landfills. The Forward Landfill Preliminary Closure/Post-Closure Maintenance Plan (PCPCMP), dated May 2002, was originally prepared and certified by Bryan A. Stirrat & Associates (see Section 6.0) in accordance with 27 CCR, Chapters 3 and 4 and 40 CFR, Part 258. The following PCPCMP revisions have been included as Amendment No. 4 (dated July 2008) as prepared by SWT Engineering: Cover/Spine (Replacement) Table of Contents (TOC), pages i, ii, iii, and iv (Replacement) (Discard previous page v of the TOC) Pages 1-5 and 1-6 (Replacement) Page 3-16 (Replacement) Pages 5-1 and 5-2 (Replacement) Section 6.0-A Professional Certification of Accuracy — Amendment No. 4 (Addendum) Table 2 — Diminishing Landfill Capacity (Replacement) Table 8 — Closure Cost Estimate Summary (Replacement) Table 9 — Annual Post -Closure Maintenance and Monitoring Cost Estimate Summary (Replacement) Appendix B — Financial Assurance Documentation (Replacement) These revisions, which comprise Amendment No. 4 of the PCPCMP, are hereby certified by Mr. Michael A.
